Welcome!
WELCOME TO STREETERVILLE PEDIATRICS!
Welcome to Streeterville Pediatrics, we’re glad you’ve chosen us to help you care for your child. In this sheet we’ll give some “nuts and bolts” info for you. Please visit our website for more complete information about our office, doctors, general medical information, and announcements:
Enroll Your Child in Your Insurance!
It is your responsibility to sign your child up to your insurance plan as soon as possible. Failure to do so promptly may cause you to go through tons of extra paperwork and phone calls – what a hassle! Let us know of any insurance changes.
Also, look at the section "Important Insurance Questions" to understand how your insurance works (co-pays, coverage for which services, etc). Please take the time to review and understand this now, as it will help to decrease your out of pocket expenses!
Please note that co-pays must be paid at the time of the visit.
Routine Telephone Calls:
Feel free to ask routine questions during your office visit. However, if a problem arises between visits that requires attention, give our office a call. We will try to return your routine call as soon as possible, understanding that we have to give priority to phone calls regarding sick children. If you've called and not received a return call, please call leaving an evening telephone number so we may reach you later.
After Hours Emergency:
We call forward our same telephone number to an answering service at night; one of the doctors on call will be paged. Please try to reserve this time for truly worrisome calls. We are happy to answer all other calls during office hours. If you call during office hours and things have changed, please call back. We try to return calls based on severity, please note that the doctors often return non-urgent calls after office hours.
Making Appointments / Seeking Advice:
Please call our number 312/280-1480 during regular office hours (M-F 9-4:30, SAT 9-12) to set up appointments for “well-child” or “sick” exams. There may be times when there are a heavy number of calls and if you seem to be waiting too long for a routine type of call, please try again later; for something more urgent, please stay on the line.
Sick Visits: Please note that we will always try to see your child that day if he or she is sick. We will do our best to have you see your chosen doctor but cannot assure this.
Well Child Visits: As our schedule can fill up rather quickly, be prepared to make your child’s next appointment before you leave the office. This will enable you to have your best selection.
Please see our website for routine type of medical information and links to other information.
Tardy & Cancelled Appointments:
Please try to be on time for your appointment as disruptions in the flow can cause the physicians to run late. If you arrive more than 20 minutes late, we will attempt to fit you in, however, the appointment may have to be rescheduled.
If you know that you are going to be late or have to cancel, please let us know as soon as possible so that we may offer time to those who are sick. We attempt to confirm appointments by phone the day before; however, we are not able to do this in all cases. Please keep track of your scheduled appointments.
Parking:
Please see our website for more info. Parking is available in our building (partially discounted – the receptionist can stamp your ticket) and at the Northwestern lots (larger discount). For Northwestern lots, you must pay us first (cash only, sorry!) then we will give you a voucher to present to exit the lot.
