New Patient Forms / Info

Please read and completely fill out the attached new patient forms.  Having paperwork finished upon arrival helps to ensure that you are seen in a timely fashion.  Please remember to bring your current insurance card and a photo identification card (i.e.: drivers license) for your visit.


Patient Registration Form - please print, fill out, and bring with you for your visit.  If you've already filled out page 1 while in the hospital, just bring in page 2.

Immunization Registry
This form is to allow your child's immunizations to go onto the State Registry- please review this  - we will ask you to sign these forms electronically when you arrive.

HIPAA Notice of Privacy Policy
By law, we need to have a signed acknowledgement of receipt of HIPAA Privacy Rules - please review this  - we will ask you to sign these forms electronically when you arrive.

Financial Policy - please review this; we will have you electronically sign this form when you are at the office

Important Insurance Questions - Please read this to make sure you are aware of what your insurance carrier may or may not cover!  You do not need to bring this with you to the office - it is just good info for you to be aware of.
