Insurance Plans

What a mess!  Things in the health insurance world have been changing so rapidly.  Perhaps the best thing to do is to CALL your insurance plan or Human Resources Department to check on eligilbity.  We have learned that many things listed on various insurance plan websites are VERY wrong (outdated information, wrong provider names, participating providers not even listed or searchable, etc).

Please CALL your insurance company (do not rely on their website!!) to determine participation. In order to make sure that we are contracted with your insurance carrier, you can provide them with the Streeterville Pediatrics Tax ID: 363535360 or NPI - National Provider ID: 1225070691 (if they can't find us listed by name!).

With the continuing changes in the Healthcare Insurance market, it is more important than ever that patients are familiar with their personal coverage/benefit plans. 

Please understand that we contract with insurnace companies through the hospitals that we are on staff with.  It has happened from time to time that the hospital has terminated contracting with an insurance company - even without our knowing.  We do try to verify eligibility for parents, however this system has so many breakdowns.  Please, always be aware of your healthcare coverage!
