Here is a convenient listing of vaccines and administration times, presented in two different formats:
Vaccine Schedule by Vaccine Type
HBV - Hepatitis B
Birth, 1m, 6m or later.
DTap - Diphtheria, Tetanus, (acellular) Pertussis
2m, 4m, 6m, 18m, 4y.
Tdap (similar to DTaP above)
for older children at 10-12 years
IPV - Polio
2m, 4m, 6-18m, 4y.
HIB - Hemophilis influenza B
2m, 4m, 6m, 12-18m.
Prevnar - PCV13 - Pneumococcal
2m, 4m, 6m, 12-18m.
Rotateq - Rotavirus (oral)
2m, 4m, 6m
HAV - Hepatitis A
12m, 18m-24m
MMR - Measles, Mumps, Rubella
12-15m, 4-5y.
Varivax - Varicella (Chicken Pox)
12-15m, 4-5y.
HPV - Human Papillomavirus Gardasil
9-26 years old: 2 doses given at least 6 months apart (if started at an earlier age); otherwise 3 shots if started late
MCV4 - Menactra
11y and 16y
Meningitis B - Trumenba
16y and second dose at least 6m after first
Influenza
after 6 months of age: 2 doses one month apart, then yearly in Fall.
