ADD/ADDH Vanderbilt Evaluation Forms

 & Medication Policy

If you've been directed to this area of our website by your doctor, these are the forms that you will need to complete so that you your child's situation can be evaluated.

Please download the following forms and fill them out.  You can give copies to as many people as you think necessary - teachers, grandparents, babysitters, coaches - whoever sees your child frequently.  Please send these forms into your doctor so that we can evaluate them for you.

Vanderbilt Form - Teacher

After your child has begun on a treatment strategy for at least two weeks, you may then want to fill out the following "Follow-Up" evaluations to gauge how well your child is doing on treatment. You may want to look at and fill out these "Follow-Up" forms every few weeks or months to assess your child's progress.
