Consent to Treat for Your Child

Here is a sample form to use if you are going to be out of town or unavailable.  You may download and fill the form out. You do not have to have it on file at our practice.  Please have  your child's guardian have it in his/her possession.  Note that you will need a witness to sign this form also.
